You can either print this information off as a complete pack or you can print the information off individually. See below.

    This information form will give you further information regarding the below. Click here to download the document.

  • Registration Checklist, Click here to download the document.(All psychotherapists need to complete this checklist and send it into the Board along with your supporting documentation). You will also need to complete the following appendices where necessary.

  • Template - Character Reference/Professional Reference, (APPENDIX ONE) Click here to download the document.

  • Template - Declaration - Supervised Psychotherapy & Therapeutic Hours, (APPENDIX TWO) Click here to download the document. (Psychotherapists applying for the Psychotherapist Scope of Practice or the Psychotherapists Scope of Practice with Child and Adolescent Specialism will need to complete this form)

    How do I move up from the Interim Psychotherapist Scope of Practice?
     

    To move from a Interim Psychotherapist Scope of Practice to a Psychotherapist Scope of Practice or Psychotherapist Scope of Practice with Child and Adolescent Psychotheraipst Specialism you will need to complete the following:

    a) (APPENDIX SIX)Template - Declaration, Supervised Psychotherapy and Personal Therapy Hours Form Click here to download the file. Request to move from the Interim Psychotherapist Scope of Practice to the Psychotherapist Scope of Practice

    b) (APPENDIX SEVEN) Template - Declaration, Supervisor Click here to download the file. Request to move from the Interim Psychotherapist Scope of Practice to the Psychotherapist Scope of Practice/Psychotherapist Scope of Practice with Child and Adolescent Specialism Supervisor Sign off

    (c) (APPENDIX EIGHT) Template - Declaration - Assessment, Click here to download the file.
    This declaration applies to those without a master's level qualification in psychotherapy or child psychotherapy. Applicants are required to complete a Board approved assessment of clinical and professional practice.
