To move from an Interim Psychotherapist Scope of Practice to a Psychotherapist Scope of Practice or Psychotherapist Scope of Practice with Child and Adolescent Psychotherapist Specialism you will need to complete the following:

(c) (APPENDIX EIGHT) Template - Declaration - Assessment, Click here to download the document. This declaration applies to those without a master's level qualification in psychotherapy or child psychotherapy. Applicants are required to complete a Board approved assessment of clinical and professional practice.

Clinical supervision can be provided by either:
1. A registered psychotherapist with a current APC; or
2. A health practitioner registered under the HPCAA with a current APC, approved by the Board as having sufficient psychotherapy training, knowledge, skills and professional development.

This form is used to assist the Assessors determining health practitioners who fall into category 2 above. In completing this form you are seeking appointment as a 'primary' clinical psychotherapist supervisor. In addition to this completed form practitioners also need to forward to the Board a current CV.
